As a Care Co-ordinator youll join a team who are focused ondelivering impeccable customer service to our staff and service users to reallymake our service users and staff feel the difference.

HCRG Care Group provides community services across the WestLancashire area, with the Single Point of Access Team being an integral part ofthis service. As a Care Co-ordinator, you will provide excellent customerservice to patients, GPs, etc who access this community hub.

Main duties of the job

To provide an excellent customer experience for patients and professionals contacting the Single Point of Access (SPA) Team by telephone and email as the first point of contact to HCRG Care Group Community services in West Lancashire. Ensuring effective filtering and directing telephone calls for services across Community services in West Lancashire and where required with partner organisations.

Supporting clinical teams by booking and scheduling routine clinic activity based on rules and protocols and signposting other activity to the relevant services within required timescales.

The CCH is operational 8am 8pm, 7 days a week
