Overview

The associate continuing healthcare practitioner was approved for delivery on 11/07/2019. This Higher apprenticeship standard awards you with a level 5, qualification when completed. The usual duration of this apprenticeship is 24 months, but this does not include the End-Point Assessment Period.

What the course covers

The Associate continuing healthcare practitioner apprenticeship has been designed to provide apprentices with the experience to plan and commission on-going care packages to people with complex health and social needs, required as a result of disability, accident or illness.

You will communicate with individuals and their family, friends and carers and be their first point of contact as you offer support, care, and advice about the service you will be providing. You will carry out reviews of individuals needs and create care plans, understanding the principles of medicine and infection control. You will also be responsible for assessing patients circumstances and screening them for eligibility in line with the National Framework for Continuing Healthcare, and its related legislation and policies.

You will need to be prepared to work shifts, commonly including both evening and weekends, and be reliable and consistent in your approach as you show discretion and self-awareness. You will also have to have excellent personal skills, as you communicate sensitive information and guide patients through potentially emotional and stressful treatments, acting as an ambassador for the healthcare industry.
