Job summary
A unique opportunity has arisen to complete a 2 year Continuing Healthcare Apprenticeship leading to a professional qualification of Associate Continuing Healthcare Practitioner.
Following completion of the apprenticeship the post holder will be responsible for assisting in the delivery of patient centred assessments to identify any primary health care need which may result in eligibility for NHS CHC (or associated funding stream) ensuring the national framework for NHS Continuing Healthcare and NHS-funded Nursing Care is implemented and complied in accordance with the principles and processes.
Associate Continuing Healthcare Practitioner Entry Qualifications
English and Math s – Apprentices without level 2 English and maths will need to achieve this level prior to taking the End-Point Assessment. For those with an education, health and care plan or a legacy statement, the apprenticeships English and maths minimum requirement is Entry Level 3. A British Sign Language (BSL) qualification is an alternative to the English qualification for those whose primary language is BSL.
Study Days Apprentices are expected to attend weekly sessions both in person and virtually with the training partner, these are expected to be a full day on a Thursday and may involve travel nationally

- Can you discuss sensitive health conditions with families and clients
- Are you willing to work hard and study hard
